  In the matter of the original invalid pension claim No. 116004, Burnett Hoard, Co. F, 152nd Inf. Ind.
  On this 6th day of July A.D. 1893 personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Burnett Hoard, aged 72 years, a resident of Conklin in the County of Ottawa and State of Mich. well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforesaid case as follows:
  My post office address is Conklin, Mich.  For 30 years immediately preceding my enlistment into the service of the United States on the 27th day of Feb, 1865 I resided in the following named places: Lagrange, Ind. and that my occupation has been that of a farmer.
  Since my discharge from said service, on the 30th day of Aug, 1865 I have resided in:  Lagrange Ind. 65-66; Paw Paw, Mich 66-69; Kalamazoo, Mich 69-70; Lagrange, Ind 70-78; Conklin, Mich 78-95, and that my occupation has been that of a farmer.
  I further state that the disability for which I claim a pension arises from Rheumatism which was contracted at  _______Point, East Virginia, in August of 1865.  I was in no hospital and had no medical treatment in service; but, was off duty on account of it some 5 weeks as to this circumstance I submit an affidavit of Capt. J. H. Catvin & Lt. Hobson is dead & Lt. Bennett I cannot find....
  From my said discharge to the present time I have received the following medical treatment for said disease:  Dr. White, Lagrange, Ind. 1865 for diaherea but not for rheumatism; Dr ______, 1865 (he is dead); Dr. Wilkinson, 73-83, I do not know his address; Dr. Beach, Conklin 1892 ..., I had no other Doctor.  But, to continue, I _____ _____ of Myron Blodjett & wife & ______  ______ on 15 years.  Batson & Green 12 years.
  Since the origin of the disability for which pension is claimed, I have sufered with the following acute diseases:
Diaherea & piles & injury to testicles past 15 years and I do not know what caused it.  And also right hernia; I do not know what caused it.  I also had a severe blow on the forehead when a boy from kick of horse.  I cannot furnish any further evidence as to my circumstance of injury to testicles nor hernia, nor head; nor can I furnish anymore medical evidence on the rheumatism.   

Affidavit to Origin of Disability
  In the matter of the Pension claim of Burnett Hoard in Co. F, 152nd Reg't, Ind., Inf., Vols., personally came before me a Justice of the Peace in the aforsaid County and State, JAMES H CATVN, of Mount Pisgah, County of Lagrange, State of Indiana, who, being duly sworn declares in relation to the aforesaid claim that his age is 54 years; that he is the identical person who served as Captain in Co. F, 152nd Reg't, Ind., Vols., and knows the above soldier, who was a member of Co., F, 152nd Reg't, Ind., Inf.; that on or about June or July ...  of 1865, while in the line of duty, and without fault or improper conduct on his part, at or near ________ Point, State of East Virginia said soldier incurred Rheumatism.
  I was Captain of Co F, 152 Regt, Ind, Vols, ... and I was well aquainted with said claimant while in said service and remember well that on or about June or July 1865 at our area near ________Point (East Virginia) said claimant contracted Rheumatism caused by exposure in heavy rain storms while on guard duty which was insident to the life of a soldier.  I often excused the said claimant from duty as he was very lame and suffered very much from said disabiliy.
  And on or about sometime in August said claimant contracted ______ in his back by lifting up a log which was in camp, this was in camp Pratt in West Virginia.
  This affidavit was written by myself and I was not aided or dictated by any other person than myself and I know all the facts as I had command of the company at that time.    Affiant further declares that he has no interest, direct or indirect, in this claim, and that he makes the above statement from personal knowledge. Affiant's Post Office address is as follows: Mt Pisga, LaGrange Co., Ind.     

Physician's Affidavit
  State of Mich, County of Ottawa, ss:
In the pension claim No 116.004 of Burnett Hoard late of Co "F", 152nd, Ind., Inf. personally came before me a Notary Public in and for the aforesaid county and State, Henry Beach, M.D. a citizen of Ravenna Township whose post-office address is Conklin, Ottawa Co., Mich well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to the aforesaid case as follows:
  That he is a praticing physician, and that he has been acquainted with said soldier for about one year and that he did not know the soldier until one year ago since that time has been his family physician and has lived one half mile from said soldier and has seen him often and knows him well both as a neighbor and an M.D..  Was called to treat said soldier in May of 1892 and found him suffering terribly with chronic rheumatism and prescribed for him then and has done so frequently since.  And also find him a sufferer from _______hernia and considers said soldier total disabled from performing any kind of manual labor.  signed: Henry Beach, M.D.  April 12, 1893

Physician's Affidavit
State of Indiana
LaGrange County, ss:
  Before me personally came E G White, M.D. who being by me first duly sworn on oath says that I am well acquainted with Burnett Hoard who was a private in company "F" of the 152nd regiment of Indana Vols. and was called and attended his family himself as the family physician and knows that before going into the 152nd he was a stout healthy man not aflicted with any kidney disease or diarrhea.  That after his discharge from the 152nd Ind. Vols. he was very much afflicted with chronic diarrhea causing a weakness of his back that I was frequently called upon to prescribe and did prescribe medication for him for the chronic diarrhea.  That from the time of his discharge until the fall of 1867 he was almost constantly afflicted with the chronic diarrhea to such an extent as he was almost entirely disabled from
______ caused by the diarrhea ... he was still suffering from the disease & in my opinion he is incurable.  He removed from this county about the fall of 1867.  Since then I have not mailed him, I am not a relative & am not invested in this claim for pension ...

General Affidavit

Personally appeared before the undersigned duly authorized to administer oaths within and for the said County, Mrs. Annie Blodget & Myron Blodget whos P.O. is Conklin, County of Ottawa, State of Mich, who being duly sworn, states in relation to said claim as follows to wit:

I was at Burnett Hoards when he came home from the army and he complained about having the rumatis and I have been acquainted with him ever since and he has always complained more or less about the rumatis and I have known of his _______ for the rumatis and he was not able to do any work for over one year after he came home from the army and he is still a constant sufferer from the rumatis.  This statement is from 1865 to 1890
And affiant further states that he has no interest in this claim.

Vols.  Personally appeared before the undersigned duly authorized to administer oaths within and for the said County, Myron Blodgett aged ___ years whose P.O. is Conklin, County of Ottawa, State of Mich., who being duly sworn, states in relation to said claim as follows to wit:

I have bin personally acquainted with Burnett Hoard ever since he came home from the army and he has always complained about having the rumatis, and he has often complained about his head feeling so disey ever since the year of 1865 till the present time and he aint bin able to do only light work and about one half of the time he hasn't bin able to work on account of the rumatis and he has bin a sober industrous man and is worthy of this affidavit.
And affiant further states that he has no interest in this claim.
